can anyone point me in the right direction, I have tried something already but it didnt work, so suggestions welcome!

I want a note "where the red line is" saying what the last topic was and who the poster was

I have downloaded and followed the changes of code on the text doc, and saved changes, but it hasnt changed

is there anything else I need to do?

You just have to follow the change and replace the original file by the new one.

You can also try to empty the cache of your browser.

replace the file? there wasnt anything to replace it with - it asks you to edit the existing index.php file by finding and replacing code, this I did and then saved changes, with no visible change

I am new to all of this, perhaps I am missing something - ?

You have to drop the file on your FTP after making change. It will replace the existing one.
